Driver medicals
C1 and D1 driver medical
For ambulances, minibuses and smaller lorries, recorded on the DVLA D4.
- Form
- DVLA D4
- Appointment
- About 15 minutes
- Price, eye test included
- £55

Who needs this medical
C1 covers medium sized goods vehicles, which includes most ambulances. D1 covers minibuses. Both are Group 2 categories, so the DVLA asks for a D4.
Patient transport and community transport employers often want a current medical before you start. We'll complete the D4 and any employer form you bring along.
Usually booked for
- Ambulance and patient transport drivers
- Minibus and school transport drivers
- Drivers adding C1 or D1 to their licence
What's included
- An identity and driving licence check
- A look through your medical history and medications
- An eye test
- Blood pressure, pulse, height and weight
- The physical examination your form asks for
- Your form filled in and signed before you leave
How it works
Step 1: Book online
Choose your medical and a time. Evening and weekend slots sit alongside weekday ones. For a council medical, pick your licensing authority and we'll have its form ready.
Step 2: Come to the clinic
Bring your driving licence, your glasses if you wear them, and a list of any medication you take. Most driver medicals take about 15 minutes.
Step 3: Leave with your form
Your doctor fills in the form, signs it and hands it to you before you go.

What to bring
- Your driving licence
- Your glasses or contact lenses, if you wear them
- A list of your current medications
If your employer has its own form, bring it and we'll fill it in alongside the D4.
Questions
Do I need to bring the form?
Usually not. We keep the forms for 67 licensing authorities as well as the DVLA D4. Do bring any letter your council or the DVLA has sent you, so we can check which version applies.
How long does the appointment take?
Most driver medicals take about 15 minutes. Allow a bit longer if you need two forms filled in, or if there's a lot in your medical history.
Will you tell me the outcome at the appointment?
Your doctor will go through what has been recorded before you leave. The outcome depends on the examination, so nobody can tell you what it will be in advance.
What if something is found that needs following up?
Your doctor will explain what has been recorded and what it means for your application. If more information is needed, from your own GP or a specialist for example, they'll tell you exactly what.
