Frequently asked questions
Straight answers to what drivers ask us most, from what to bring to how the form is filled in.

Ask the clinic
Tell us which council or licence it's for and we'll make sure the right form is ready.
The appointment
How long does the appointment take?
Most driver medicals take about 15 minutes. Allow a bit longer if you need two forms filled in, or if there's a lot in your medical history.
What should I bring?
Your driving licence, your glasses or contact lenses if you wear them, and a list of any medication you take.
Do you have evening or weekend appointments?
Yes. You'll see evening and weekend times alongside weekday ones when you book online.
Where is the clinic?
Lime Square Medical Centre, Ashton Old Road, Manchester M11 1DA.
Do I need to be registered with you?
No. We're a private clinic and see any driver, whichever GP practice they're registered with.
Forms
Do I get the form on the day?
Yes. The doctor fills in and signs the form during your appointment, and you take it with you.
Do I need to bring the form?
Usually not. We keep the forms for 67 licensing authorities as well as the DVLA D4. Do bring any letter your council or the DVLA has sent you, so we can check which version applies.
Can you complete two councils' forms in one visit?
Yes. One examination covers both, and the second council's form is £45.
The medical
Who carries out the medical?
One of our GMC registered GPs. The doctor who examines you is the one who fills in and signs your form.
Will you tell me the outcome at the appointment?
Your doctor will go through what has been recorded before you leave. The outcome depends on the examination, so nobody can tell you what it will be in advance.
What if something is found that needs following up?
Your doctor will explain what has been recorded and what it means for your application. If more information is needed, from your own GP or a specialist for example, they'll tell you exactly what.
Prices and payment
Is the eye test included?
Yes. Every driver medical includes the eye test.
What does it cost?
Every driver medical is £55, eye test included. A fit to fly assessment is £100 and a private GP appointment is £60. If you need a second council form at the same visit, that is £45.
How do I pay?
By card when you book, or at the clinic on the day. It costs the same either way. If your employer is paying, get in touch first.
Can you invoice my employer or fleet?
Yes. We set up an account for the company before the first appointment, then invoice its drivers' medicals together.
Ready to book?
Pick a time online, including evenings and weekends. Pay when you book or at the clinic.
